For the Love of Art Nomination Form - Graphic Design

    To nominate a student for the Graphic Design program, complete and submit the below form. You will receive an automated confirmation email upon submitting. 

    Once we process the nomination (please allow up to 5 business days), the nominated student will receive an email detailing next steps and program information. A copy of the email will be forwarded to the nominator. 

    *all fields below are required, including student date of birth. 
    *to nominate multiple students, you will need to complete a separate form for each student.
