For the Love of Art Nomination Form - Graphic Design

    To nominate a student for the Fall 2019 Graphic Design program, complete and submit the below form. You will receive an automated confirmation email upon submitting. 

    Once we process the nomination (please allow up to 5 business days), the nominated student will receive an email detailing next steps and program information. A copy of the email will be forwarded to the nominator. 

    *all fields below are required, including student date of birth. 
    *to nominate multiple students, you will need to complete a separate form for each student.
